1.  How to complete the Application Form:

  • Insert the NUMBER OF A-class SHARES that  you would like to apply for in the section marked A. The minimum number is one hundred

  • Funds lodged with the Application must agree with the amount in the section marked B. (e.g. Section A: two hundred shares; Section B: $200.00

  • Insert the full name that you wish to appear on the Share Register, including full first names (not initials) in the section marked C. This must be your own name our the name of a company.

  • You may enter your Tax File Number (TFN) or exemption category under your name in the section marked D. Quotation of your TFN is not compulsory however Kids 4 Kids Inc. is obliged to deduct income tax at the top marginal rate if it is not disclosed.

  • If you are applying as Joint Applicants, complete the section marked E.

  • Enter your Postal Address for correspondence in the section marked F.

  • Insert your telephone number and contact name in the section marked G for contact regarding your application

  • If you are sending a cheque, insert cheque details in the section marked H. Cheques are to be made payable to Kids 4 Kids Inc. and marked "Account Payee Only".

  • If you are applying and using the on-line facility for MasterCard or Visa , click on the link to our secure server.

3.  Correct Forms of Name to Register:

 

Note that only legally recognised entities are allowed to hold shares. Applications must be in the name(s) of natural persons, companies or other legal entities acceptable to Kids 4 Kids Inc. At least one name given in full with the surname is required for each natural person.
